INSURANCE |
The majority of office visits are
fee-for-service, which means you will receive an itemized receipt for services
rendered. Payment is expected at the time of visit.
If you are a member
of a managed care plan, or have been injured at work (Worker's Compensation) or
an automobile accident (No Fault) you will be required to provide us with all
requested insurance information including any necessary referrals prior to any
treatment being rendered. Some managed care plans require a referral from your
primary care physician before we can treat you, so we suggest that you check
your individual plan first.
Dr. Napoli participates in many major health
plans including Medicare, HMO's and PPO's, however, you are responsible for any
co-payments, deductibles and non-covered services.
For your convenience,
payment may be made by cash, check or credit card. We accept American Express,
Visa and Mastercard.
